To notify a claim to Furness Underwriting, please identify the claim category and:
Click on the "Online" button and complete the process following the guided procedure.

Alternatively:

1. Click on the "PDF" button.
2. Print, complete, and sign the form.
3. Send the form and documentation to claims@furnessunderwriting.com
